Illuminated Direction LLC,
Is a mental and behavioral health company that specializes in providing wrap around services for adults and children. Illuminated Directions’ main focus is to help individuals reach their ultimate potential by achieving more control over their personal lives. Illuminated Direction promotes self-improvement through the use of skill building activities done through community interactions, as well as office-based services that are focused on self-advocacy. Illuminated Direction’s goal is to encourage the individual to take the steps towards taking responsibly and understanding the role they play in being successful and independent in all aspects of their lives.

Illuminated Direction mission is to improve communities through bring an awareness of mental and behavioral health by promoting solution-based practices to reduce recidivism, hospitalization, out of home placement, truancy, and homelessness.
Services
Psychiatric Rehabilitation Program (PRP)
PRP services provide rehabilitation and support for participants to develop and enhance their community and independent living skills. Services may be provided at a PRP facility (onsite) at a residence, job, or another appropriate location in the community (off-site); or at a combination of the two onsite as well as off-site support.

Mental Health Skill Building services are defined as individualized training to enable individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environments.
Illuminated Directions provides training in the following areas:
- Functional Skills and appropriate behaviors relating to the individual’s health and safety, activities of daily living, and use of community resources.
- Assistance with medication management
- Monitoring health, nutrition, and physical condition.
Individuals that qualify for the service muse demonstrate a clinical necessity for the service arising from a condition due to mental, behavioral, or emotional illness.
Service Criteria as Follows:
- Having a need for individualized training in acquiring basic living skills such as symptom management.
- Adherence to psychiatric and medication treatment plans.
- Development and appropriate use of social skills and personal support system.
- Personal Hygiene
- Food Preparation
- Money Management Training
- Having a Qualifying Mental Health Diagnosis (From a licensed mental health professional)
- Having a history of a qualifying mental health condition
Intensive In Home
Services are goal directed training to stabilize the home environment through bringing awareness of mental health diagnosis by the use of skill building activities to prevent out of home placement.
Criteria for Program Participation
- Impairments experienced by the member are to such a degree for being at risk of out of home placement
- Having difficulty in establishing or maintaining normal interpersonal relationships
- Inappropriate behavior that documented, repeated interventions by the mental health, social services or judicial system are or have been necessary resulting in being at risk for out of home placement.
- Extreme difficulty in cognitive ability such that they are unable to recognize personal danger or recognize significantly inappropriate social behavior resulting in being at risk for out of home placement.
- Transitioning out of residential treatment facility or hospital back into the home environment.
